2) The conclusion of the Academic information centre on the equating of the acquired education to the level required for the learning in the relevant programme, if the previous education is obtained abroad. Please, find more information about this document in the Admission Rules for the relevant programme (section Admission Rules of the website)
3) A copy of a document certifying a change of name, surname, personal code (if applicable)
